I'm not sure exactly what your looking for as far as a return of the form but I used this site - http://phpformgen.sourceforge.net/ With almost no knowledge of php I was able to create a form online at their site then download the finished code. I uploaded it to the server and followed the directions given to get it going. The only other thing I did was rename the folders since they showed in the address. I don't know if you can insert the code into an existing webpage as yours is now, though. The two I created are independent .php pages but they do include a comment page after submission that you can customize as well as a redirect link back to your home page or wherever. Hope this helps
