PHP Forms

I have a form in http://www.ranvet.com.au/thoroughbreddiet.htm that returns to us via email. The form as shown on the web page is not delivered, simply a vertical list of the information.

This is time consuming for those in our company working with the returns. I wish for a form exactly as displayed( and many other variotions) to be returned as emai attachments or forms??????

I am led to believe that a PHP form is needed and as we have zero skills in PHP I need someone or some software (idiot proof) to provide the solution.

Any help greatly appreciated

I'm not sure exactly what your looking for as far as a return of the form but I used this site - http://phpformgen.sourceforge.net/

With almost no knowledge of php I was able to create a form online at their site then download the finished code. I uploaded it to the server and followed the directions given to get it going. The only other thing I did was rename the folders since they showed in the address.

I don't know if you can insert the code into an existing webpage as yours is now, though. The two I created are independent .php pages but they do include a comment page after submission that you can customize as well as a redirect link back to your home page or wherever.
